Minor Participant Equine Activity Waiver & Parental Consent (NJ)

This Agreement applies solely to participation in the Heritage Ranch Summer Day Camp and is entered into by the undersigned parent or legal guardian (“Parent/Guardian”) on behalf of the minor participant (“Minor”), in favor of Heritage Ranch LLC, its owners, employees, instructors, volunteers, agents, and representatives (collectively, “Released Parties”).

ASSUMPTION OF RISK

I understand that participation in horseback riding, horsemanship, animal care, and ranch activities involves inherent risks, including the unpredictable behavior of horses and farm animals, surface conditions, weather, equipment, and the actions or omissions of others. On behalf of the Minor, I knowingly and voluntarily assume all such risks.

SAFETY APPAREL & HELMET DISCLAIMER

I acknowledge that the Minor must wear proper riding apparel while mounted, including a properly fitted equestrian helmet, closed-toe shoes or riding boots, and long pants. I understand that helmets do not guarantee protection against injury or death and voluntarily accept all risks related to helmet use or non-use.

CONDUCT SUPERVISION

I understand that the Minor must follow all rules and instructions provided by staff at all times and remain under direct supervision. I acknowledge that unsafe, disruptive, or non-compliant behavior—by the Minor or Parent/Guardian—may result in immediate dismissal from camp without refund, in whole or in part, at the sole discretion of Heritage Ranch.

RELEASE AND INDEMNIFICATION

To the fullest extent permitted by New Jersey law, I release, waive, and agree not to sue the Released Parties for any claims arising out of the Minor’s participation in the Summer Day Camp, including claims resulting from the ordinary negligence of the Released Parties. I further agree to indemnify and hold harmless the Released Parties from any claims arising from the Minor’s participation.

MEDICAL AUTHORIZATION

In the event of an emergency, I authorize Heritage Ranch to obtain medical treatment for the Minor and accept responsibility for all related costs.

CANCELLATION POLICY

The $100 deposit is a non-refundable placeholder that is applied to your overall balance.

• After May 1, full tuition required at signup.

• After June 1, tuition is non-refundable if cancelled by the parent or legal guardian.

• If cancelled prior to June 1, tuition will be returned minus the $100 deposit.

New Jersey Equine Activity Liability Act Notice

WARNING: Under the New Jersey Equine Activity Liability Act (N.J.S.A. 5:15-1 et seq.), an equine activity sponsor or equine professional is not liable for injury to or death of a participant resulting from the inherent risks of equine activities, pursuant to N.J.S.A. 5:15-5.

ACKNOWLEDGEMENT & GOVERNING LAW

This Agreement shall be governed by the laws of the State of New Jersey. By submitting this form, I certify that I am the Parent/Guardian of the Minor, have read and understand this Agreement, and agree to its terms.
